Terms & Conditions & Health Assessment Waiver/Physical Activity Readiness Questionnaire (PAR-Q).
In order to participate each team member must complete and sign the Terms & Conditions, Waiver which is the Physical Activity Readiness Questionnaire - PAR-Q (required by the Gym).
